Medical Records Requests
Sunshine Clinic is committed to protecting your privacy while ensuring you have convenient access to your medical information when you need it. Patients may request a copy of their medical records at any time by following the process outlined below.
Requesting a Copy of Your Medical Records
1. Complete a Medical Records Request Form
To begin the process, patients must complete a Medical Records Release Authorization Form. This form allows Sunshine Clinic to verify your identity and ensures that records are only released with your permission. The form may be requested from our office or downloaded from our website.
2. Submit Your Request
Once completed, the authorization form may be submitted to Sunshine Clinic through one of the following methods:
• In person at one of our clinic locations
• By email to our administrative office
• By fax
• By mail
Please be sure the form is fully completed and signed to avoid delays in processing.
3. Identity Verification
To protect patient privacy and comply with HIPAA regulations, Sunshine Clinic may require a valid photo ID or additional verification before records are released.
4. Processing Your Request
After your request is received and verified, our staff will begin processing your records. Most requests are completed within 7–10 business days, though more complex requests may take additional time depending on the size of the record.
5. Delivery of Records
Medical records can be provided in several formats based on your preference:
• Secure electronic delivery
• Encrypted email
• Paper copiesSu
• Fax transmission to another healthcare provider
6. Possible Fees
In accordance with state guidelines, a reasonable administrative fee may apply for printed copies of medical records.
